Add blog URL display to elsevier-harvard.csl

Add blogs to also display URL and access date.
As per guidelines:
"Web references
As a minimum, the full URL should be given and the date when the reference was last accessed. Any further information, if known (DOI, author names, dates, reference to a source publication, etc.), should also be given. Web references can be listed separately (e.g., after the reference list) under a different heading if desired, or can be included in the reference list."

Update modern-humanities-research-association.csl for artwork

Changed et-al-min to 3 at first use

JMS leaves this pretty ambiguous. If et al starts right away and at how many authors is not defined in their style guide. However, it was pointed out to me that all other articles in JMS use et al right away, at the 3rd author, so that's what I changed the style to.
