Style guide for chapters #2

I propose following loose guidelines so we don't have to spend time and effort in reformatting:

Chapter title is H1 heading (in markdown this is a single #)
Chapter sections are H3 headings
Subsections are H4

Open question - How should we handle references / citations?

I think we can use the style of chapters 1 and 2 at this point..
